TEMPLE CHAI CAPITAL CAMPAIGN PLEDGE FORM
We invite you to make a financial commitment towards our new home and be part of Temple Chai’s exciting future!
Honor Wall
Those making a commitment of at least $3,600 will be included on the honor wall in our new home. You may choose to use your own name, or to dedicate your donation in honor/memory of a loved one.

Make a commitment of $100 for 36 months to ensure your spot on the Honor Wall
This can be made in payments of $100 per month for 36 months. 
You will be able to split your payment into monthly payments on the next screen after you hit "submit."
We will reach out to confirm the details and ensure your recognition is exactly as you wish.
Mezuzah Society
Join the Mezuzah Society with a special contribution. As a member, you can select the location of your mezuzah, honor a loved one, celebrate your family, and share a meaningful ritual with Temple Chai clergy to affix your mezuzah to its doorway.

This payment can be made in a one-time commitment of $18,000 or $500 per month for 36 months. 
You will be able to split your payment into monthly payments on the next screen after you hit "submit."
We will contact you to discuss the details of the placement of your mezuzah
Other Financial Contribution
If you prefer to make a smaller contribution, we appreciate any amount you can give to support Temple Chai’s Moving Forward Together Capital Campaign. Every donation, no matter the size, makes a meaningful impact.
Additional Naming Opportunities:
There are many unique opportunities for you to honor or memorialize a loved one between the gardens, classrooms, offices and other areas of the new campus.

By signing this Pledge form, I/we acknowledge and agree that my/our gift is subject to and bound by Temple Chai’s Gift Acceptance policy (available upon request) and Attachment A, which is attached to this Pledge form (together, the “Gift Terms”).
By signing this Pledge form, I/we confirm my/our gift is intended for the sole use of Temple Chai’s Moving Forward Together Capital Campaign as stated in the Gift Term. I/We understand the Pledge being made may not be used to offset an existing pledge, become part of another future campaign, or be transferred except as set forth in the Gift Terms. Recognition will only apply to Temple Chai’s Moving Forward Together Capital Campaign
This form and Gift Terms represents our entire agreement, superseding any/all prior understandings, oral or written, express or implied, with respect to this Pledge and is legally binding under the laws of the State of Arizona
